Client Rights
Clients at our community clinic have several important rights. Clients are entitled to:
1. The right to request health records maintained by the Baylor Center for Disability and Flourishing:
To request records, send a written request, either through email (bcdf@baylor.edu) or by mailing or delivering the request in person at our community clinic (2201 MacArthur Drive, Suite 101, Waco, TX).
Please include your full name, your child’s full name (if applicable), the client’s date of birth, and a description of the records you are requesting. Your request will be processed in accordance with applicable Texas law and federal privacy regulations (HIPAA).
2. The right to contact the licensing authority:
You may contact the appropriate Texas licensing authority to ask questions about licensure or professional standards:
